Hospice Buffalo, along with 1000 other national, state and community organizations, is promoting National Healthcare Decisions Day (#NHDD) April 16. National Healthcare Decisions Day is a collaborative effort of organizations committed to ensuring that all adults with decision-making capacity have the information and resources to communicate and document their future healthcare decisions. Advance care planning involves making future healthcare decisions that include much more than deciding what care you would or would not want; it starts with expressing care preferences, clarifying values and beliefs and selecting an agent (healthcare proxy) to voice your healthcare decisions if you are unable to do so.

Advance care planning starts by talking with your loved ones and your healthcare providers. These conversations will relieve loved ones and healthcare professionals of the need to guess what you would want if faced with a medical crisis. As a result of National Healthcare Decisions Day, more people in our community can be expected to have thoughtful conversations about their healthcare wishes. If you have not had conversations with your physician and loved ones, please do so today! It is our goal to encourage everyone 18 years and older to designate a heathcare agent and document their healthcare decisions by completing a New York State Healthcare Proxy form.
